Applicants, residents, parents, guardians, and authorized representatives can use the resident portal to apply for Residential Habilitation Services, update information, upload documents, and communicate with LiftUp staff.
If you have trouble registering, signing in, updating your history, uploading DSPD/Medicaid documents, or viewing your care information, contact LiftUp Health Center for assistance.
Please do not send sensitive health information through ordinary email unless directed by LiftUp staff. Use the secure resident portal for health, disability, medication, DSPD, Medicaid, and other protected information.
